Hospice of the Northwest was known as Skagit Hospice Services when it started in 1984. Skagit Hospice was officially formed as a Limited Liability Company (LLC) with services in Skagit, Island, San Juan, and Snohomish counties.

In 2003, Public Hospital District #1, now known as Skagit Regional Health, and Public Hospital District #304 entered into an operating agreement to jointly support the hospice service. They renamed the organization Hospice of the Northwest and continue to jointly operate the organization today.
The operating agreement for Hospice of the Northwest defines the LLC’s members as the Commissioners of the two hospital districts, as well as their Superintendent or Chief Executive Officer. To ensure adequate representation of these members in operational decisions for Hospice of the Northwest,

the Superintendent of each hospital district assigns two senior administrators to sit on an Operations Council.
These four Operations Council members volunteer their time to ensure Hospice of the Northwest runs smoothly, efficiently, and sustainably. They meet monthly to review the financial health and quality metrics for Hospice of the Northwest, as well as to discuss operational opportunities and new initiatives.
The Hospice of the Northwest Foundation was founded in 2001 and is a separate 501(c)3 organization governed by a volunteer Board of Trustees.
